Telephonic Case Manager RN Medical or Oncology - Remote id-9706

As a Telephonic Case Manager RN, you’ll play a vital role in coordinating care, providing patient education, and supporting individuals through their health journey—all from the comfort of your home.
Responsibilities
- Conduct telephonic assessments to evaluate patients' medical conditions, treatment plans, and care needs.
- Collaborate with healthcare providers, specialists, and support teams to ensure seamless care coordination.
- Provide education and resources to patients and caregivers to enhance self-management and improve health outcomes.
- Develop and implement personalized care plans tailored to individual patient needs.
- Monitor patient progress, address concerns, and adjust care plans as necessary.
- Advocate for patients to ensure they receive high-quality, cost-effective care.
- Document all interactions, assessments, and care plans in compliance with regulatory and company guidelines.

Requirements
- Active Registered Nurse (RN) license in the United States.
- Minimum of 3 years of clinical nursing experience, preferably in medical-surgical, case management, or oncology.
- Experience in case management, care coordination, or utilization review is highly desirable.
- Strong communication, problem-solving, and critical thinking skills.
- Ability to work independently in a remote environment with minimal supervision.
- Proficiency in electronic medical records (EMR) and telehealth technologies.
