LTSS Service Coordinator-RN Clinician id-11601
About the position
The LTSS Service Coordinator-RN Clinician is responsible for managing members' cases within the scope of licensure, providing supervision to non-RN clinicians, and developing and revising care plans to optimize member health care. This role involves performing clinical assessments, coordinating care for members with chronic illnesses, and ensuring access to necessary services.
Responsibilities
- Manage overall case management for members within the scope of licensure.
- Supervise and direct non-RN clinicians involved in member care.
- Develop, monitor, evaluate, and revise member care plans.
- Perform face-to-face clinical assessments to identify and manage member needs.
- Coordinate care for members with chronic illnesses and disabilities.
- Establish short and long-term goals in collaboration with members and their support systems.
- Assist with the implementation of member care plans and facilitate authorizations/referrals.
- Interface with Medical Directors and Inter-Disciplinary Teams on care management plans.
- Supervise the work of LPNs, LSWs, and other licensed professionals in coordinating services.

Requirements
- Current, unrestricted RN license in applicable state(s).
- Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or Case Management role.
