Key Responsibilities:<br />
• Conduct Annual Wellness Visits with a focus on patient-centered care.<br />
• Develop & implement care plans for individuals with chronic illnesses.<br />
• Diagnose and treat non-emergency conditions efficiently.<br />
• Oversee medical treatment plans for remotely monitored patients.<br />
• Collaborate with RCP Nurse Coaches to promote wellness & prevent urgent care or emergency visits.

• Manage the care of high-risk and rising-risk patients in coordination with PCP/Specialists.<br />
• Refer patients to specialized professional services as needed.<br />
• Build & maintain strong patient relationships through education and coaching.<br />
• Integrate seamlessly with the practice QHP and support staff.<br />
• Respond promptly to triaged patient situations by RCP Nurse Coaches.<br />
• Participate in daily operational huddles with nursing and support staff.<br />
• Serve as a medical knowledge resource to RCP Nurse Coaches.<br />
<br />
Required Experience:<br />
• APRN (Nurse Practitioner) with a minimum of 5years experience in outpatient or primary care setting.<br />
• Preferred focus in Family, Adult, or Gerontology.<br />
• National Certification required<br />
• Active & Unencumbered RN & APRN license<br />
• Proficient with EMR systems & computer technology; high-speed internet required.<br />
• An enterprising and progressive mindset<br />

Location: This is a fully remote/virutual position.<br />

Job Details:<br />
• Full-time, salary position (85k-90k).<br />
• Practice hours maintained.<br />
