Job Title: Registered Nurse (RN)
The Diverse Home Care Services Inc. is seeking a qualified Registered Nurse (RN) to join our team. As a registered nurse, you will plan, organize and direct home care services for patients in need of nursing assistance.
About the Role
We are looking for a highly motivated and compassionate RN who can work independently with minimal supervision. You will be responsible for providing patient care, developing care plans, and communicating with healthcare professionals and family members.
Key Responsibilities:
- Complete initial assessments of patients and their families to determine home care needs
- Provide physical assessments and histories of current and previous illnesses
- Develop and implement care plans based on patient needs and goals
- Counsel patients and their families on meeting nursing and related needs
- Administer medications and treatments as prescribed by physicians
- Communicate with healthcare professionals and family members regarding patient progress and changes in condition

Requirements:
- Current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Current CPR certification
- Licensed driver with an automobile that is insured in accordance with state or organization requirements
- One year experience in hospital or home care nursing preferred
- Self-directed and able to work with minimal supervision
Work Environment:
This role involves working in a home care setting, where you will be required to travel to patients' homes and work independently. You will also be required to participate in on-call duties as defined by the on-call policy.
