Responsibilities:
• Provide virtual support and assistance to patients and caregivers through telehealth chat services.
• Conduct patient assessments and gather relevant medical information to assist healthcare professionals in making informed decisions.
• Respond to patient inquiries and provide education on health conditions, treatment plans, and medication management.
• Monitor patient vitals and alert healthcare professionals of any significant changes.
• Coordinate and schedule telehealth appointments for patients.
• Maintain accurate and detailed records of patient interactions and progress.
• Collaborate with the healthcare team to ensure seamless communication and continuity of care.
• Keep up-to-date with telehealth technology and industry advancements to provide the best support to our patients.
Requirements:
• Bachelor's degree in a healthcare-related field or, equivalent work experience.
• At least 2 years of experience in a healthcare setting, preferably in telehealth or virtual care.
• Excellent communication and interpersonal skills.
• Proficient in telehealth technology and software.
• Strong organizational skills and attention to detail.
• Ability to work independently and in a team environment.
• Compassionate attitude towards patient care.
• Experience with home health care is preferred.
• Must have a valid driver's license and reliable transportation.
