About the position
The Registered Nurse (RN) position at YAI/National Institute for People with Disabilities involves providing essential nursing services to individuals supported in assigned programs. The RN will collaborate closely with direct support staff and program leadership to gather information and updates on the health status of individuals, ensuring that all necessary observations and assessments are conducted on-site.
This role requires the collection of vital data, including vital signs, social history, and current health concerns, to support comprehensive assessments of the individuals in care. The RN is responsible for alerting community physicians to any significant health complaints, particularly those that may pose life-threatening risks, and must document all interactions and observations in accordance with established guidelines and regulatory requirements. In addition to direct patient care, the RN will utilize the nursing process to assess the health conditions and educational needs of individuals, providing necessary education regarding treatments, medications, and healthy lifestyle choices.
The role also includes triaging emergency and non-emergency calls from program staff and individuals, recommending appropriate actions, and documenting these interactions according to established protocols. The RN will maintain a thorough understanding of relevant regulations, including Titles 17 and 22, and will be responsible for writing and updating individualized nursing health care plans for the individuals supported. The RN will participate in multidisciplinary team meetings, presenting health information and responding to inquiries regarding the status of individuals in care. They will also be involved in training direct care staff on medical needs and medication administration, ensuring compliance with all relevant regulations, including HIPAA. The position requires flexibility, as the RN may need to travel between assigned programs and respond to medical concerns outside of regular working hours. Overall, this role is critical in ensuring the health and well-being of individuals with disabilities, requiring a combination of clinical expertise, communication skills, and regulatory knowledge.

Responsibilities
- Provide nursing services for persons supported in assigned programs.
- Collaborate with direct support staff and program leadership to gather health information.

- Utilize the nursing process to assess health conditions and provide education.
- Triage emergency and non-emergency calls from staff and individuals.
- Observe, record, and report patient conditions and reactions to treatments.
- Develop and maintain understanding of relevant regulations (Titles 17 and 22).
- Write and update individualized nursing health care plans.
- Present health information in multidisciplinary team meetings.
- Train direct care staff on medical needs and medication administration.
- Participate in intake assessments and review medical records.
- Interface with external medical providers to coordinate care.
- Comply with all regulatory agency requirements, including HIPAA.
Requirements
- Current active RN license issued by the State of California Board of Registered Nursing.
- Current Basic Life Support (BLS) certification for Healthcare Providers.
- Two years of experience performing professional nursing duties, preferably with individuals with I/DD or mental health needs.
- Demonstrated sound clinical judgment in resolving medical concerns.
- Excellent interpersonal, verbal, and written communication skills.
- Proficiency with Microsoft Office and electronic health records.
- Highly organized and flexible, able to manage multiple priorities.
- Ability to travel between assigned programs as needed.
- Ability to respond to medical concerns outside of regular working hours.
- Ability to meet essential physical demands of the position.
Nice-to-haves
- Familiarity with California's Lanterman Act and Title 17 and 22 regulations.
- Two or more years of RN experience working with individuals with I/DD in a residential setting.
